How to Order Ocular Tissue from Southern Eye Bank
Welcome to our team! Southern Eye Bank prides itself on distributing ocular tissue to surgeons who need corneal tissue, sclera patches, or education and research tissue. Because of the generosity of Louisianans, Southern Eye Bank is can provide this gift to those in need.
To request ocular tissue, fax a surgical request to 504-891-2401. After our team reviews your request, we will verify your order within 24 hours.
You may also call our office at 504-891-3937. We are staffed 24/7.
You may print a copy of the Surgical Tissue Request Form by clicking on “Surgical Tissue Request Form.”
Print the completed form, and fax it to 504-891-2401.
To expedite your request, please provide the following information:
- patient name
- unique identifying number,
- sex
- race
- date of birth
- address
- name of performing surgeon
- name of surgery center or hospital
- address of surgery center or hospital
- diagnosis
- which eye requires surgery
Please let us know how to prepare the tissue:
- pre-cut corneal tissue for use in Endothelial Keratoplasty
- full-thickness cornea for use in Penetrating Keratoplasty
- half/whole sclera shells
For surgeries in the United States, our technicians will call the day before the scheduled surgery. They will provide the surgery coordinator (or surgeon) with tissue specifications. We usually call at approximately 3 pm CST.
For international tissue distribution or Monday morning procedures, our technicians will call in advance to discuss shipping arrangements.
